Sometimes when people have very specific treatment needs, they need an individual placement to be purchased for them which could be close to home or could be in another part of the country.

This is often because the treatment is "specialist" and is provided for at a regional or national level.

Referrals for individual patient treatments are usually made by a consultant or lead clinician.

This usually happens because the need is very unusual or the type of service can only be provided on a scale at regional or national level.
 
Usually, the requests are for specific mental health treatments, acquired brain injury or gender dysphoria, but can include other things.
 
In Plymouth - the decisions on these treatments are made by NHS NEW Devon CCG.

In Devon - decisions are made by NHS NEW Devon CCG and Devon Partnership NHS Trust.

Both areas hold panels that receive applications and make decisions.
