NHS continuing healthcare (CHC)

CHC is the process for determining eligibility for NHS funded care for people who are either receiving long-term care or who may be at the end of their life and need palliative care.

This process is nationally defined and we follow a standard process and use standard information collection tools to decide whether someone is eligible.

From April 2014, anyone eligible for CHC will also be able to receive the money they need as a Personal Health Budget in the form of a direct payment. This will give people greater choice and control over their care and support.

Services we commission

We commission individual packages of care and support in care homes and in people’s own homes.

In addition, NHS NEW Devon CCG is responsible for ensuring the quality of care and ensuring that the national process - NHS CHC is followed properly.

Who are these services for?
CHC services are for people who, because of primary health needs, are eligible for NHS CHC funding.

This includes people with physical disabilities, learning disabilities and older adults with complex physical and psychological needs.

What is the function of the services?
NHS CHC ensures that people with complex health needs have their care and support funded by the NHS.

How to access the services?
To access NHS CHC, people need to be screened and assessed by a nurse.

In Plymouth this assessment is provided by staff employed by Plymouth Community Healthcare.

For the rest of Devon - this assessment is provided by Northern Devon Healthcare NHS Trust or Devon Partnership NHS Trust.

If you feel that you, a family member or someone you care for are eligible for NHS CHC, then you can speak to your community nurse or your GP about an assessment.

Refreshed Redress Guidance

NHS England has published draft refreshed NHS CHC Redress Guidance for CCGs, to be used when considering NHS CHC redress payments for individuals. 

The document has been updated to reflect the 'Principles for Remedy' from the Parliamentary and Health Service Ombudsman (PHSO) for all public bodies.
